Our Single/Couple Direct Primary Care (DPC) package is designed to provide high-quality, integrative care through convenient virtual Telehealth services. Here’s what you can expect from our family-centered approach to health and wellness: $179 per month, or less than $90 per person. Services include: -Unlimited virtual/telemedicine visits -Access to personal cell phone for urgent needs -Exclusive add-on option for personalized in-home visits, including newborn wellness checks (surrounding Charlotte Metro area only) -Holistic/integrative care for the whole family -Personalized support from our functional nutritionist -Acute and Minor Sick Visits: Covering conditions such as strep, colds, viruses, URI, UTIs, GI symptoms, women's and men's urinary health, musculoskeletal concerns, mental health, hormones, skin issues, etc. -Telehealth Wellness Checkups: Regular virtual checkups to monitor overall health and wellness. -Routine Disease Management: Ongoing care and monitoring of chronic conditions like diabetes, hypertension, asthma, etc. -Long COVID Treatment/Ivermectin -Medical and natural weight loss -Coordination of Specialty Care/Referrals: Assisting patients in getting the specialized care they need. -Discounted wholesale lab work and discounted scans -Holistic/integrative approach to any acute or chronic needs -Diet/Lifestyle Counseling/Disease Prevention: Helping patients with diet, lifestyle changes, and preventive care. -Bioidentical hormone replacement therapy (gels, creams, oral, patches only) -Family Planning and Reproductive Health: Virtual consultations for contraception, fertility, and reproductive health. Additional Information: *Payments & Cancelation: All payments are processed monthly using the credit card provided at registration. By enrolling, you agree to a 6-month membership commitment. If you wish to cancel, we kindly ask for a 30-day written notice prior to your next billing cycle. *Interested in signing up or have questions? Cancellation Policy
Appointment and Membership Policies Appointment Cancellations: Members can cancel and reschedule their appointments as needed. To ensure smooth scheduling and availability for all members, please cancel or reschedule appointments at least 24 hours in advance. Membership Cancellations: We value your membership and understand that circumstances may change. Members have the freedom to cancel their memberships anytime after the 6 month commitment for family DPC memberships . To help us process your cancellation smoothly, we request a 30-day written notice before your intended cancellation date. This ensures that we can finalize any necessary administrative tasks and provide a seamless transition. Scheduling: As a valued member, you have the advantage of scheduling appointments up to 4 weeks in advance. This allows for greater flexibility and convenience in planning your healthcare needs. Please note that the latest an appointment can be booked is 24 hours prior to the desired time. This policy ensures that we can accommodate all our members efficiently and effectively.
